About the GMR Toolkit
Who we are

Why this site exists
This site is a collection of instruments created by our group to measure health. Each tool has a short description, the questionnaire and its result, and the references to cite.
Privacy
- The tools run in your web browser. We do not collect, receive or store your answers. This site uses no cookies, analytics or tracking.
- While you work, your answers are kept only in this browser tab, so reloading the page does not lose them. They stay until you close the tab or choose New assessment, which clears the answers to every tool in this tab. Going back in the browser does not bring them back. Some browsers can bring them back if you reopen a closed tab. On a shared computer, choose New assessment and close the browser when you finish.
- Downloaded files and printouts contain your answers. Keep them private. If you keep a copy about a patient, follow your organization’s rules for patient information.
- Copy for the chart puts one line about the result on your device’s clipboard. The line contains the result but no name, because the tools never ask for one. Some computers and phones keep a clipboard history or share the clipboard with your other devices. If yours does, the line can stay in that history or reach those devices. On a shared or work computer, follow your organization’s rules.
- The tools never ask for a name or health card number. Please do not send patient details by email.
- This site is hosted by GitHub. Like most websites, GitHub keeps a record of visitors’ IP addresses for security. We cannot see these records. GitHub’s privacy statement
